I, the undersigned, hereby confirm that: • I am the authorised representative of the deceased and have the legal right to instruct Heritage Cremations (Pty) Ltd to proceed with the direct cremation. • I have read, understood, and fully accept the Terms & Conditions – Direct Cremation Services provided by Heritage Cremations (Pty) Ltd. • I understand that full payment is required before the cremation can take place, and that all outstanding balances must be settled before the release of ashes. • I acknowledge that additional fees may apply, including but not limited to storage, mileage, statutory documentation, embalming (if required), and extended distance or international courier services. • I understand that Heritage Cremations may, in exceptional circumstances and at their sole discretion, approve a structured payment plan, and that any such agreement will be legally binding. • I accept that Heritage Cremations reserves the right to pursue legal action, including court proceedings or any other lawful recovery methods, for any unpaid or outstanding invoices, including those arising from a breach of an approved payment plan. • I acknowledge that Heritage Cremations is not responsible for delays caused by third party providers, government departments, industrial action, load shedding, or any circumstances beyond their reasonable control, and that additional storage or related fees may apply in such cases. By signing below, I confirm my full acceptance of these terms and authorise Heritage Cremations (Pty) Ltd to proceed.
